Requirements:


Diploma in Social Services, Community Services, Healthcare Administration, Business Administration, or related fields


Minimum 2 years' experience in social service agencies, healthcare, community care or other related social service settings


 


Responsibilities:

Referrals and Intake Coordination

Manage referrals, including:


Processing referrals


Updating status from assignment to discharge


Preparing client profiles


Scheduling intake (Day Care) and Initial Assessment (Home Therapy) and providing brief system updates


Updating progress notes and liaising with AIC officers where required


Supporting social workers in simple Financial Assistance (FA) documentation and submissions, financial deviations


Registration and Admission

Manage admission and data entry, including:


Service Agreements (Sharing with seniors and signing of agreements), other required forms


Coordinate client registrations


Flag follow-up needs for Social Work support


Care Plan (ICP) / MDT Support


Provide documentation and administrative support to Social Workers for MDT processes and conduct simple questionnaires where required (with training)


Support discharge processes and financial billing and invoicing matters


Any other assigned matters related to administration and documentation, day to day operations
